LifeLong Medical Care is looking for a Case Management Coordinator to provide leadership, supervision and management to LifeLong’s Supportive Housing Project (SHP) Case Managers. Major responsibilities include supporting SHP case management teams, providing administrative and personnel oversight of case managers, developing and implementing a trauma informed case management model, attending collaborative meetings with property management, housing providers and other stakeholders, overseeing client care processes, assuring quality data collection and service documentation, and monitoring quality of services.

The Case Management Coordinator is a key team member and collaborator with the SHP Director, Lead LCSW and the SHP Services Manager.
