Organization Overview:

Kaleidoscope Family Solutions, Inc. provides community and home-based services to persons with developmental and intellectual disabilities. Our team partners with family members and existing providers to create individualized goals allowing the team to help improve their life experiences.

We are looking for a dedicated and committed Community Support Worker who strives to make a difference in the lives of the individuals we serve.

Responsibilities:

The primary role of a Community Support Worker is to help individuals acquire and maintain skills to improve their daily-living, domestic, adaptive, and socialization skills.

Provide 1:1 In-Home and Community Based Support to individuals.

Assist individuals with achieving a variety of goals in the home and the local community.

Transport individuals to medical appointments, personal shopping, and recreation activities.

Cultivate safe and supportive relationships.

Complete and Submit accurate weekly progress notes.

Complete necessary training based on the Individuals’ needs.

Requirements:

Minimal of a High School Diploma or General Equivalency Diploma (GED).

Must have a valid Driver’s License, Auto Insurance + Vehicle Registration.

Background check.

Must be willing to drive Individuals in the personal car throughout the community.
